UnityPoint at Home (515) 574-6416– Home Health Care has nurses, nurse aides, social workers and therapists who visit you in your home. You can choose from a list of home health care agencies if you need one when you go home.

UnityPoint Palliative Care (515) 574-8519– Palliative care is the treatment and relief of long term illness. During your illness, you and your family may need care and support. Trinity’s Palliative Care services can help you manage your illness.

UnityPoint Hospice (515) 574-6416 – Hospice services help to improve the life for the dying patient and their families. Trinity Hospice offers staff to visit with patients and family members about hospice services. Hospice care can take place in the patient’s home, nursing home or assisted living center.

UnityPoint at Home Hospice (515) 332-2492 - The hospice team for Humboldt County consists of HCMH staff, UnityPoint at Home Hospice team and the Humboldt County Hospice Foundation.

UnityPoint Medical Equipment Services (515) 574-8540 – Provides a broad choice of home medical equipment, products, services, and needed training. The store is located inside the main entrance of Trinity, near the gift shop.
