Walk-In Sport Physicals

The UnityPoint Clinic - Humboldt (located within the Humboldt County Memorial Hospital) will be hosting walk-in sports physicals. Parents are urged to be with their child at the time of the appointment or be available by phone . Make sure to have your child's updated insurance information with you (them) when they come in for the physical. 

The following days will be secured for walk-in sports physicals: 

• Tuesday, July 16th from 8AM to 11:30AM and 1PM to 4:15PM
• Thursday, July 18th from 8AM to 11:30AM and 1PM to 4:15PM
• Wednesday, July 24th from 8AM to 11:30AM and 1PM to 4:15PM

 

The paperwork below needs to be filled out prior to being brought back into the exam room. Parents need to give consent for vaccines treatment, etc. which is why we urge the parents to either be with their child at the time of their appointment or be available via phone.
